1 INDICATIONS AND USAGE ZOHYDRO® ER (hydrocodone bitartrate) is used to treat pain that is severe enough to require daily, around-the-clock opioid medication for a lengthy period of time and for which other treatments are ineffective. Order 15 mg Zohydro ER oral capsule extended release online

As-needed (prn) analgesia is not recommended with ZOHYDRO ER.
2 ADMINISTRATION AND DOSAGE
2.1 Initial Dosing ZOHYDRO ER should only be given by healthcare experts who are familiar with the use of powerful opioids for chronic pain management.
Only patients who have built tolerance to an opioid of comparable potency should use ZOHYDRO ER 50 mg capsules, a single dose of ZOHYDRO ER greater than 40 mg, or a total daily dose greater than 80 mg. Patients who have received at least 60 mg oral morphine per day, 25 mcg transdermal fentanyl per hour, 30 mg oral oxycodone per day, 8 mg oral hydromorphone per day, 25 mg oral oxymorphone per day, or an equianalgesic dose of another opioid for one week or longer are considered opioid tolerant.
Start the dose regimen for each patient separately, taking into account the patient’s prior analgesic treatment experience as well as the patient’s risk factors for addiction, abuse, and misuse. Patients should be continuously monitored for respiratory depression, especially in the first 24-72 hours after starting ZOHYDRO ER medication.
The entire dose of ZOHYDRO ER is required [see PATIENT COUNSELING INFORMATION (17)]. Crushing, chewing, or dissolving the pellets in ZOHYDRO ER capsules causes uncontrolled hydrocodone distribution, which can result in overdose or death.
Use of ZOHYDRO ER as the First Opioid Analgesic Start with one 10 mg capsule of ZOHYDRO ER every 12 hours.
In Patients Who Are Opioid-Intolerant, ZOHYDRO ER Is Used
ZOHYDRO ER 10 mg orally every 12 hours is the beginning dose for those who are not opioid tolerant. Patients who have received at least 60 mg oral morphine per day, 25 mcg transdermal fentanyl per hour, 30 mg oral oxycodone per day, 8 mg oral hydromorphone per day, 25 mg oral oxymorphone per day, or an equianalgesic dose of another opioid for one week or longer are considered opioid tolerant.
In patients who are not opioid tolerant, using higher initial doses can lead to deadly respiratory depression.
Other Oral Opioids to ZOHYDRO ER Conversion
When you start ZOHYDRO ER therapy, you should stop taking any other opioids that you use all the time.
While there are tables of opioid equivalents freely available, the relative efficacy of different opioid medicines and products varies significantly from patient to patient. As a result, it is preferable to underestimate a patient’s 24-hour oral hydrocodone requirements and offer rescue medication (e.g., an immediate-release opioid) than to overestimate the 24-hour oral hydrocodone requirements and risk unpleasant effects. Patients were converted from their prior opioid to ZOHYDRO ER in a ZOHYDRO ER clinical trial with an open label titration period, utilizing Table 1 as a guidance for the initial ZOHYDRO ER dose.
